At the Spine and Brain Institute of San Diego, we value your time, which is why we have recently split our New Patient Packet into two parts. This allows all patients more time to fill out each section at their leisure, without the additional stress of not being able to save and/or go back to edit. All patients now have the option to fill the second section out online or in the office when they come in for their first visit. Please note that should you decide to fill it out online, once you start the process, you will have to continue until its completion, otherwise we will not receive anything on our end.
