Detailed steps to download and print this form
Download the PDF
- Locate the toolbar displayed above the form
- Locate the folder icon
- Click on the folder icon and the form will automatically be downloaded
Open the PDF
- Go to your “Downloads” folder where the PDF was saved
- Locate the PDF file named “NP-Packet-Updated-xx.xx.xx” (The Xs indicate a date, the date which the form was last updated)
